Job Summary

Responsibilities

  • Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary.
  • Monitor and document vital signs, medical records, and patient progress accurately within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems while maintaining strict compliance with HIPAA regulations.
  • Support patients with medication administration following prescribed schedules and medical terminology guidelines.
  • Observe patient conditions closely, noting changes in health status or behavior related to dementia, Alzheimer’s disease, or other cognitive impairments such as dementia or memory care needs.
  • Provide specialized care for individuals with developmental disabilities or those receiving hospice care by implementing tailored care plans.
  • Assist with household tasks including cleaning, laundry, meal preparation, and ensuring a safe environment conducive to recovery or long-term care.
  • Use medical equipment such as catheters or Hoyer lifts safely and effectively while adhering to all safety protocols.
  • Collaborate with healthcare professionals to update medical documentation and ensure continuity of care across home & community settings.

Skills

  • Strong patient observation skills to detect subtle changes in health or behavior promptly.
  • Proven experience in assisted living facilities, nursing homes, or home health environments.
  • Familiarity with EMR/EHR systems for accurate medical documentation and record keeping.
  • Knowledge of assisting with ADLs including bathing, dressing, grooming, meal prep, and light housekeeping tasks.
  • Experience working with individuals with disabilities or cognitive impairments such as Alzheimer’s disease and dementia.
  • Ability to perform vital signs measurement and basic medical procedures confidently and accurately.
  • Excellent communication skills to effectively coordinate with healthcare teams while respecting patient privacy under HIPAA guidelines.
