Job Summary
This position is responsible for supporting in-person clinic visits, including communicating with patients prior to their visit and helping guide patients through the care experience. The Care Team Assistant gathers pertinent medical information from the patient and documents the information into the patient’s medical records. This role also helps to ensure the exam rooms are stocked, equipment and supplies are available and assists with tasks that support the effectiveness and efficiency of the team. The Care Team Assistant may also assist in scheduling and coordinating appointments, preparing schedules, and other delegated duties as assigned.
Compensation
We are committed to providing wage ranges for all open positions. Please note that the specific compensation for this role will be determined based on experience, education and internal equity considerations. The hourly wage range for this position is $19.49–$27.51. This range reflects the base salary for this position.
Key Accountabilities
- Prepare patients for their appointments, including communicating with patients in advance of their scheduled appointment as needed to ensure medical records are collected, received and available for visits if applicable.
- Perform chart prep and pre-visit planning as defined by specific clinic processes in preparation for patient visit appointments.
- Assist in the rooming process at the time of the in-person visit by collecting and documenting pertinent information such as the reason for their visit, patient reported medication review, vitals collection with automated vitals machine and applicable health questionnaires.
- Prepare exam rooms and stock supplies.
- Communicates effectively with patients, providers and other health professionals, demonstrating comfort in person and with virtual technologies.
- Checking in and checking out patients for visits which may include scheduling follow-up appointments.
Required Qualifications
- High School graduate or equivalent.
- Current BLS must be enrolled in NH+C RQI program and complete the requirements within 90 days of enrollment. Must remain current (complete quarterly assignments prior to the end of each calendar quarter).
- Ability to learn and use electronic health records and other computer programs.
- Must be able to speak, read, and write English proficiently to ensure safe and effective communication in a healthcare environment.
Preferred Qualifications
- Previous work in healthcare settings.
- One year of customer service experience.
- Proficient in Medical Terminology.
- Proficient Working with an Electronic Health Record.
