Title:
 Care Coordinator

Status: Full-time; Hourly, Non-exempt

Compensation: $25.00 – $27.00 per hour; depending on experience (specialty, clinic volume, front office preferred)

  • Responsible for the tracking and coordinating of all referrals.
  • Contact and schedule patients for all incoming referrals and provide notification back to referring provider.
  • Facilitate all outgoing referrals, ensuring timely exchange of clinical documentation between providers.
  • Process and coordinate outgoing referrals for diagnostic imaging.
  • Verify referral accuracy, medical necessity and supporting clinical documentation.
  • Coordinate and schedule surgical procedures in collaboration with physicians, surgical facilities, anesthesia departments and clinical staff.
  • Verify provider orders, procedure details, and patient eligibility prior to scheduling
  • Obtain and track prior authorizations for surgical procedures as required by insurance carriers.
  • Communicate pre-operative instructions, appointment details, and preparation requirements to patients.
  • Manage surgical schedule changes, cancellations and rescheduling while minimizing disruptions to patient care.
  • Ensures that incoming and outgoing referrals are addressed in the outlined time parameters.
  • Communicate effectively with physicians, APP’s, medical assistants and specialists.
  • Ensure complete and accurate registration, including patient demographics and current insurance information.
  • Promote collaboration within the facility to improve patient access to care and patient outcomes to increase the efficiency of the team.
  • Navigate and maintain accurate records across multiple EMR and scheduling systems.
